RIGHT TO APPEALThe benefits provided by the FA Benefit Fund may be changed by the Board of Trustees. The Board of Trustees adopts rules and regulations for the payment of benefits and all provisions of this booklet are subject to such rules and regulations and to the Trust Indenture which established and governs the FA Benefit Fund operations. All rules are uniformly applied by the Fund Office. The action of the Fund Office is subject only to review by the Board of Trustees. A member or beneficiary may request a review of action by submitting notice in writing to the Board of Trustees, Faculty Association of Suffolk Community College Benefit Fund, Suffolk Community College, 533 College Road, Selden, New York 11784-2899. Such notice must be sent within 60 days from the date of the explanation of the denial and shall include any argument and evidence in support of the appeal. The Trustees shall act on the appeal within a reasonable period of time and render their decision in writing, which shall be final and conclusive and binding on all persons. BENEFITS QUESTIONS?
